Create a Question Card

There are two options for creating a Question Card form in TikTok Ads Manager.

Option 1: From the Tools tab

  1. Open TikTok Ads Manager.

  2. Select the Tools tab, and then choose Instant Page.

  3. Select the Instant Form tab, and select the Card Layout.


Option 2: During campaign creation

  1. Open TikTok Ads Manager.

  2. Select Campaign, and then click Create.

  3. Select Lead Generation as the Advertising Objective and create a new Ad Group with Instant Form selected as the Optimization Location.

  4. Once you have set up the Ad Group, click +Create in the Destination section at the ad level to access the Instant From Builder.

  5. Choose the Card Layout.


Note: Remember to connect your CRM if you have one integrated on TikTok.

How to customize a Question Card form

Step 1. Set up the color scheme and language

  • Use the drop-down menu in the top right corner to change the color scheme and language settings.

Step 2. Create a Welcome Page

  • Set up the background image, title, description for your Question Card form.

  • You have the option to add a logo on this page.

  • Toggle on the Display time estimate option to display an estimated time of how long it takes to fill out your form (e.g. 3 minutes). This is calculated by the average time users need to fill out the form. If you have applied logic settings, the time is based on the maximum number of questions a user might need to answer.

Step 3. Add Questions

  • In the Question module, you can set up Custom and Personal Information Questions.

    • Select the Image Question type to create a Custom Question with answers displayed with images

    • Use Conditional Answer question type to dynamically show options for question based on response to previous question.

    • Personal Information Questions will be shown at the end of the Question Card for and grouped by category on a single page.

  • You have the option to set up the logic to display your Custom Questions on this page.

    • Note:

      • You need to have at least two Custom Questions to set up the logic.

      • After turning on the Logic setting, you will not be able to adjust the order of questions.

Step 4. Add Your Privacy Policy

  • In the Privacy module, add your Company Name and link to your company's Privacy Policy, beginning with http:// or https://.

  • You can add additional statements, such as marketing opt-ins and legal disclaimers, by toggling on Custom Disclaimer.

Step 5. Customize the Final Page

  • Click the Final Page tab to set up how you would like the last page of your Question Card form to look like:

    • Add a headline and description.

    • Add call-to-action buttons with a link to your website and/or link to download your app.

Step 6. Update Settings

  • Click the Settings tab to set up your Form Type, Field Names in export files, and Tracking Parameters.

    • Select a Form Type:

      • More Volume: prioritize the volume of leads, where no additional review page is added for users prior to submitting the form.

      • Higher Intent: focus on leads with higher intent by adding a review page and CAPTCHA after users click Submit on your form to verify that the lead is a real person.

    • Add custom field names if you would like to match the field names in export files with fields in your CRM system. These fields are not shown to users.

  • Add the Parameter Name and Value you want to track.

    • Tracking parameters enable advertisers to identify leads coming from a specific source. These parameters are not visible to users.
