The Form Card allows you to add a simple form directly into the CTA of an in-feed video. This pre-click form experience can help you generate a higher volume of leads by accelerating form submissions through a simplified sign up experience.
Note:
If a user clicks anywhere on the Form Card outside the input box, they will be directed straight to the Instant Form.
To streamline form submissions, contact information such as name, email address, and phone number may be pre-filled for users who have previously shared their details with TikTok.
The Form Card format is a good solution for advertisers who:
Prioritize high volume of leads over high quality of leads.
Use shorter and simple Instant Forms.
To use the Form Card format, no additional set up is required on TikTok Ads Manager. However, make sure your form meets the following criteria:
Limit to 3 questions or fewer and can only include Personal Information questions (with any combination of name, email, and phone number).
Or
Begin with a multiple-choice custom question.
Note: The Form Card is not available for:
Creative format: Carousel
Instant Form type: High intent form
Instant Form feature: Logic Settings 2.0
Interactive add ons: Display card/inventory card
If the form has multiple CTA buttons on the final page or has a View content CTA button
If the form has Custom Disclaimer in the privacy policy section
